Exploring the influence of physical activity on the path to experiencing positive psychological growth in older adolescent and young adult cancer survivors

Consent Form

This study seeks to understand positive psychological growth experiences among adolescent and young adult cancer survivors. The purpose of this investigation is to explore more generally how individuals feel about their levels of social support, stress and coping, goal-setting, and positive and negative emotional experiences. Furthermore, we are interested in understanding if physical activity impacts these factors. This study is being conducted as part of a Master’s thesis.

If you are between the ages of 18-39 and have completed treatments for cancer within the last 5 years you are eligible to participate.

Your participation includes the completion of the attached questionnaire, using the webpage and details provided. The questions ask demographic information, cancer and treatment types, time passed since finishing treatment, and measures assessing: social support, goal disengagement and reengagement, coping with stress, stress levels, psychological growth and physical activity levels. The questions will take approximately 15-20 minutes to complete.

Participation in this study will help us to understand the influences of psychological growth thus increasing this positive experience among cancer survivors. There are no foreseeable risks or harm in participation.

Your participation in this study is voluntary. You may decline to answer any question as well as withdraw from the study at any time. The information you provide will be kept confidential and will not be used for any purposes outside of this research project.
